​​​​​What do I need to do to prepare for my appointment?  

 

New patients will need to bring in an updated insurance card and driver's license.  If the patient is being seen for diabetes, please bring in your glucometer for Dr. Stoever to review your blood sugar.  

 

​How do I become a new patient?

Please have your physician fax over a referral to our office along with your office notes and labs.

Do you accept self-pay patients?

Unfortunately, at this time, we are not accepting self-pay patients.
